Sanitarium

To get into fighting mode so you can fight anybody, enter (during
gameplay) I AM A KILLER.  To get all items and extra weapons,
enter IWANTAPACKUPLUNCH.

To get level select on the second chapter, go stand in the church
for about 30 minutes and you will hear somebody scream; when you
hear this, type in ISHOULDNOTBEHERE. Then go to the statue and
keep clicking until you hear a bong, then there will be extra
menus on your screen.
